What are the criteria for applying?
- The child must be 18 years or younger at the time of the application.
- The injury must have occurred after birth.
- In order to apply for your child, the child must live with you 51% or more and be listed as a dependent on your most recently filed IRS 1040. If the child is not listed on your most recently filed IRS 1040, then we need a copy of both your most recently filed IRS 1040 and the most recently filed IRS 1040 on which the child is listed as a dependent.
Is there a financial criteria?
If your family meets the financial criteria, a $5,000 annual maximum and $7,000 lifetime maximum of financial assistance applies beginning May 1, 2019.

What Happens If i don't meet the criteria?
If your family does not meet the criteria, a $2,000 lifetime maximum of financial assistance applies. If your family applies and there are insufficient funds to approve the application partially or in full, we will hold the application and review it again the following month. If your family’s application can’t be funded within 90 days, you may reapply at a later time.
